The five core agreements

Each agreement below forms part of the PharmaKind Collective contract suite.

  1. Agreement 1

    Pharmacy Client Services Agreement

    The master agreement between PharmaKind Collective and a pharmacy client for relief pharmacist coverage and related workforce support.

  2. Agreement 2

    Relief Professional Services Agreement

    The agreement governing the relationship between PharmaKind Collective and a licensed relief pharmacist providing coverage.

  3. Agreement 3

    Shift Assignment Confirmation

    A per-shift confirmation capturing the specific dates, times, location and terms of an individual coverage assignment.

  4. Agreement 4

    Preferred Pharmacy Partner Program Addendum

    An addendum for pharmacies in the preferred-partner program, setting out priority coverage and ongoing relationship terms.

    Pharmacy Consulting Services Agreement

    An agreement for pharmacy operational and consulting engagements beyond shift-based relief coverage.
